Effect of Postural Changes on Plasma Renin Activity and Aldosterone Concentration in Normotensive Subjects

Abstract
The effect of postural change from supine to standing on plasma renin activity and aldosterone concentration during low sodium intake and diuretics administration were studied in twelve normotensive young Koreans.
Normotensive young subjects were on usual Korean food, containing high sodium. Lasix was given intravenously (40mg), and blood specimen obtained at 30,60,90 and 120 min in the supine or standing posture.
Sodium intake was then reduced to 70mEq/day for four days, and blood specimens were drawn at 8:00 am every day at least after one hour in the supine position and after 20 minutes in the quiet standing, blood specimens were again taken.
Blood was promptly centrifuged and plasma was separated in cold room at 5¡ÆC.
Plasma renin activity and aldosterone concentration were measured by mean of immunoassay.
The results were as follows.
1. The plasma renin activities and aldosterone concentrations of normotensive subjects were risen on standing.
2. The plasma renin activity and aldosterone concentration were increased without significant change in postural difference during dietary sodium restriction.
3. A large rise in plasma renin activity and aldosterone concentration was observed with further enhanced postural difference following lasix injection.
4. There was a lag of 30 minute between the maximum increase in plasma renin activity and that of aldosterone following lasix injection.
5. It was discussed that plasma renin activity and aldosterone concentration as a diagnostic screening test would be better in performing in the standing position rather than recumbent position, and after lasix injection rather than dietary salt restriction.
